A Greater Effect of Posterior Cruciate Ligament Retaining versus Posterior Cruciate Ligament Sacrificing in Total Knee Replacement in Young Population – Meta-Analysis of 10 Years’ Studies

ABSTRAK
Latar Belakang: Total knee arthroplasty (TKA) adalah prosedur umum yang dilakukan terutama pada osteoarthritis lutut stadium akhir (OA). Karena posterior cruciate ligament (PCL) dapat mempengaruhi stabilitas lutut, telah diperdebatkan apakah PCL perlu dipertahankan atau dikorbankan. Penelitian ini bertujuan untuk menganalisis hasil klinis dan fungsional secara sistematis mengikuti TKA dengan pcl retaining (PCLR) dan pengorbanan (PCLS). .
Metode: Pencarian sistematis dilakukan melalui basis data online (PubMed, Google Scholar dan Cochrane Library) berdasarkan pedoman PRISMA. Studi yang melibatkan pasien yang menjalani penggantian lutut total dengan metode penahan atau pengorbanan PCL untuk OA lutut dengan tindak lanjut minimal 12 bulan disertakan. Studi non-komparatif, studi hewan dan studi non-Inggris dikecualikan. Ekstraksi data dilakukan untuk karakteristik dan hasil dasar. Review Manager (RevMan) digunakan untuk menganalisis mean difference (MD) dan odds ratio (OR) dengan interval confidence 95% (CI).
Hasil: Tujuh studi dengan total 3.346 pasien disertakan. Sekitar 2.220 pasien (66,35%) menjalani TKA dengan metode penahan PCL. Analisis mengungkapkan tidak ada perbedaan yang signifikan dalam rentang gerak (ROM), skor lutut, fleksi maksimum, WOMAC dan skor nyeri antara PCLR dan PCLS dengan nilai p masing-masing 0,10, 0,56, 0,42, 0,33 dan 0,67. Hanya skor fungsi, skor HSS dan kontraktur fleksi yang menunjukkan perbedaan signifikan dengan nilai p masing-masing 0,0002, 0,002 dan 0,004.
Kesimpulan: Analisis antara PCLR dan PCLS tidak menunjukkan perbedaan yang signifikan pada hasil klinis atau fungsional kecuali dalam skor fungsi pasca operasi, kontraktur fleksi dan skor HSS. PCLR memiliki fungsi yang jauh lebih tinggi dan skor HSS dengan kontraktur fleksi yang lebih rendah dibandingkan dengan PCLS.
