Operative Duration Predictors in Posterior Spinal Fusion for Adolescent Idiopathic Scoliosis in RSUP Dr. Hasan Sadikin, Bandung

Pendahuluan : Adolescenet idiopathic scoliosis (AIS) yang tidak ditangani dapat menyebabkan progresi kurva, nyeri punggung, dan masalah kardiopulmonal selain itu juga dapat menyebabkan isu psikososial. Intervensi operatif pada pasien AIS merupakan suatu operasi yang kompleks dan memiliki durasi yang cukup lama. Terdapat banyak variabel yang bisa menentukan panjangnya durasi operasi. Penelitian ini bertujuan untuk mengidentifikasi faktor apa saja yang dapat memprediksi variasi dalam durasi operasi posterior spinal fusion (PSF) pada pasien AIS.
Metode : 21 rekam medis pasien yang menjalani koreksi deformitas skoliosis di RSUP Dr. Hasan Sadikin antara Januari 2022 hingga Desember 2022 dianalisis secara retrospektif. Berbagai variabel yang terkait dengan pasien, prosedur, dan dokter bedah yang dinilai bisa memengaruhi durasi operasi dianalisis melalui regresi multivariat untuk mengetahui variable bebas dari prediktor durasi operasi.
Hasil : Model akhir yang diperoleh dari analisis regresi multivariat meliputi sudut Cobb dari kurva struktural utama (β = 0.35) dan jumlah sekrup yang dipasang (β = 0.28). Model tersebut dapat menjelaskan 44% variabilitas durasi operasi (R2 = 0,44). Durasi operasi memiliki korelasi yang signifikan dengan perkiraan kehilangan darah, kebutuhan transfusi darah perioperatif, dan lama tinggal di rumah sakit.
Kesimpulan : Dari beberapa variable yang memprediksi variabilitas durasi operasi selama PSF pasien pasien AIS diidentifikasi, dengan keparahan kurva menjadi prediktor terkuat.
